This report creates an evaluation sheet to be completed by attendees of a class. The class is selected from the Scheduled Class Search screen, which provides the information listed in the heading. The printed heading includes the class code, description, date, start and end times of the class, location and instructor.
Attendees can evaluate the class on various aspects of Classroom, Instructor, Training, Use of Knowledge and Additional Comments.
Report parameters include the following option:
